NWS Jacksonville:

Matthew info on front page.

On the 10th page, as of now, in the PDF file linked from that page, the "NWS Jacksonville Briefing for Hurricane Matthew", you will see values from the SLOSH model.

And also look here at the "Potential Storm Surge Flooding Map (Inundation)" map:

http://www.nhc.noaa.gov/refresh/graphics_at4+shtml/153139.shtml?inundation#contents

Of course this is an estimate. Some of the worst case values, at least worst case as of the current thinking, are about 7.7 to 8.4 feet where it looks like Neptune Beach is.

SLOSH values not specific to a storm, but a category, can be found on the Weather Underground's site:
https://www.wunderground.com/hurricane/FlAtlCoastSurge.asp
But that is dependent on a lot of things. Wind categories alone do not tell you what the surge will be, so use the images found there with caution. They are more the absolute worst case scenario I think.
